Include the page URL, the exact sentence, what you believe is wrong, and a primary source that can be checked. Screenshots help locate mobile and browser compatibility issues.

How we handle it

We first classify the issue as factual accuracy, missing boundaries, interaction failure, or reading experience. Errors affecting a core conclusion receive priority; style suggestions enter the iteration backlog.
